Bascially, whenever i hit anything (or sometimes for no reason), my hit chance drops by 60, but only once per battle.

It looks like to me your weapon is broken, but keeps repairing itself by 1 after each battle, you can repair at any blacksmith. (Michelle Gainer in Cellsvich, Yuna in the arena)
